The Basics

Cardiac healthcare professionals, including cardiologists, pulmonary specialists, or nurses commonly prescribe an exercise or rehabilitation program for patients who have had surgery, hospitalizations or specific heart-related issues. These prescriptive programs have traditionally been conducted on site in doctors’ offices, hospitals, or rehab clinics and may involve a combination of medication, strength and balance exercises, physical therapy, one-on-one guidance, nutrition and diet education, and monitoring of vital signs.

Compliance with “doctor’s orders” and participation in some type of cardiac rehab and movement have been shown to substantially reduce the risk of re-occurrence, rehospitalization, or worse. Research proves patients who partake in an exercise or rehab program are healthier, less likely to relapse, and more likely to make smarter lifestyle choices.

An estimated 65-80% of patients don’t participate due to inability to travel, financial hardship, scheduling conflicts with work or family care, or simply no nearby access. This gap is now being addressed with top-quality remote cardiac rehab.

RecoveryPlus.health is the first fully integrated and completely personalized remote, or home-based, cardiac rehab program addressing this critical void in public health. Its clinically proven program is designed for both patients recovering from surgery or hospitalization as well as those who may be at risk. We are, first and foremost, a carbon copy of in-person cardiac rehab using telehealth communications and proprietary technology. Our innovative approach to telehealth features a caring, highly experienced cardiac professional who customizes the exercise and activity of each patient to produce the best possible outcomes.

Our proprietary AI-powered telehealth platform creates a full-service, individualized home-based prehab or rehab program that tracks the patient’s progress and enables medical supervision. It includes a unique physician dashboard, patient management portal, insurance billing platform, and various patient services. There are no upfront costs or expensive administration fees for doctors to offer the RecoveryPlus.health program to their patients.

RecoveryPlus.health also offers a free app, available on iOS, PC and Android devices to demonstrate the exercise portion of the more in-depth program. The app can be used to complement participation in the full RP plan, or as a stand-alone product for people who aren’t regularly active.

The highly skilled medical team includes doctors, nurses, exercise therapists, dietitians and other professionals who work closely with each patient to motivate participation, evaluate ongoing progress and achieve success.

Within our existing network of doctors, exercise therapists, and rehab specialists, we have treated thousands of home-based patients. We have seen more than 80% of our remote cardiac rehab patients stay with the program for at least three months; more than 50% of our patients are taking preventative action to avoid future problems, and incident-recurrence rates are far below the national average.

There is no “one size fits all.” Measures of success and milestones vary for each patient within their unique, personalized program. Patients progress at different rates, and the RecoveryPlus.health platform empowers both the patient and the medical team to track these steps at an individualized rate.

If a patient is enrolled in rehab after a cardiac event, the goal is to have them return to their normal mobility/routine to avoid remission. Retention is also a key factor, and we see a huge majority of our patients engage and stay with the program.

While most in-facility rehab programs continue for several weeks to three months, RecoveryPlus.health’s at-home program allows the patient to participate for as long as they choose, or their doctor recommends. For many of our patients, remote cardiac care becomes part of an overall healthy-heart lifestyle.

What’s Covered

Patients with health insurance are covered and there are no, or only minimal, out-of-pocket costs. RecoveryPlus.health is accepted by Medicare, Medicaid, and other major insurance companies for a range of conditions including such chronic conditions as diabetes, obesity, long Covid or hypothyroidism.

Patients may need Bluetooth-enabled heartrate monitors, blood pressure cuffs, oximeters, or other devices. These may already be owned by the patient or can be supplied by RecoveryPlus.health, reimbursable by insurance. State-of-the-art technology enables patients to self-monitor or automatically track and report the patient’s progress.

Once you have been identified as having any potential risk factors or have already experienced a related medical condition, there is nothing more important than a cardiac rehab or “pre-hab” program, according to the American Heart Association and just about every cardiologist. And, yes, the RecoveryPlus platform provides this preventative solution as well.

At least half of our patients are actively participating in “pre-hab” or primary prevention activity. These patients are not prescribed traditional or facility-based rehab because they have not experienced a specific event (e.g., heart attack or surgery), but are still deemed at risk and need support to make lifestyle changes.

For Providers

  • Whether the patient is post-incident or considered at-risk, a prescription or referral from a healthcare provider is required.

  • Patient management specialists collect all pertinent information from patient/provider to create a full medical file, including diagnosis and other related conditions.
  • Any devices required by the physician can be sent directly to the patient and are reimbursable by insurance.
  • In concert with the patient’s doctor and the RecoveryPlus.health team, a personalized treatment plan of movement, diet, medication management, and other lifestyle changes is created and introduced to the patient.

The proprietary doctors’ portal is tailored for optimal patient relations management and built with an open application program interface (API), to seamlessly integrate with electronic health records and billing information. Doctors can easily review a patient’s engagement and progress at their convenience, creating billable hours and making changes to the rehab program. In this way, prescribing doctors stay informed about patients’ progress, and any “red flags” about a patient’s condition are evident sooner rather than later.

Our patients’ average monthly usage rate is about 240 minutes per month, which greatly exceeds the industry standard.
